  • Practice Information: Name of medical practice, tax identification numbers, and business contact details.

  • Personnel Information: Names, email addresses, and professional titles of individuals authorized to act on behalf of the practice.

  • Technical Data: IP addresses, browser types, and usage data collected via cookies or similar technologies to improve website performance and user experience.

2. How We Use Your Information

We use the collected information for the following purposes:

  • Enrollment and Participation: To process your application for participation in Company programs and to manage your account.

  • Service Administration: To provide management, consulting, and administrative support services as outlined in our Participation Services Agreement.

  • Communication: To send administrative updates, program information, and response to inquiries via our CRM and email platforms.

  • Supplier Coordination: To facilitate access to Covered Products and Services through our contracted suppliers and vendors.

3. Data Sharing and Disclosure

We do not sell your information to third parties. We share information only in the following circumstances:

  • Contracted Vendors and Suppliers: We may share purchasing and practice data with our contracted vendors to ensure you receive appropriate pricing, rebates, and services.

  • Legal Requirements: We may disclose information if required to do so by law or in the good faith belief that such action is necessary to comply with legal obligations.

  • Business Transfers: In the event of a merger, acquisition, or sale of assets, your information may be transferred as a business asset.

4. Data Security and HIPAA

While we do not typically collect or host patient clinical records through our public website, we maintain strict data security standards. Any Protected Health Information (PHI) handled during the course of our professional relationship is governed by the Business Associate Agreement (BAA) included in our Participation Services Agreement.

We retain your information for as long as your practice is an active participant in our programs or as needed to fulfill our legal and contractual obligations. Upon termination of our relationship, information is returned or destroyed in accordance with the terms of our Participation Services Agreement.

You may update your practice’s information or opt-out of certain communications at any time by contacting us directly. Please note that certain information is required to maintain your eligibility for program benefits and supplier pricing.
